Purpose

To identify all services offered by the agency.

The agency implements programs and services to provide comprehensive, quality health care in the patient's place of residence. All services are provided under the direction of the Administrator and in collaboration with the patient's physician.

We address each patient's physical, psychosocial, functional, and cognitive needs in an individualised plan of care, and serve as a resource to the patient and their family through health care education that keeps the patient safe at home. The goal is to treat the illness or injury, help the patient get better, regain independence, and become as self-sufficient as possible.

Objectives

  • Provide essential health and welfare services
  • Provide continuity of home health care, and the highest quality of care
  • Provide qualified skilled and non-skilled services focused on preventative, rehabilitative, and therapeutic care
  • Facilitate the patient's attainment of their optimal level of rehabilitation
  • Help the patient and family maintain as normal a lifestyle as possible
  • Augment the care provided by hospitals and nursing homes

Speech Therapy Speech-Language Pathologists (SLP)
Home Health Aide services Home Health Aides (HHA) and Certified Nursing Assistants (CNA)
Medical Social Services Medical Social Workers (MSW)

Care is generally provided for the time, duration, and frequency specified by the physician and agreed with the patient. It is delivered in the patient's residence, or in an institution or residence used as the patient's home, by agency staff and contracted professionals.

Hours

Office hours are 8:00 am to 5:00 pm, Monday through Friday.

Services are available 24/7, with an on-call nurse available 24 hours a day, 7 days a week.
