Verbal Orders and Clarifications
When you need an order, how to take one safely, and the difference between a new order and a clarification.
You can only complete interventions for which you have orders.
That is the whole rule. Everything below is how to stay on the right side of it without turning every visit into a phone call.
When you need an order
You need a new order before you:
- Change a wound treatment, dressing, or frequency
- Change visit frequency or duration, in either direction
- Add or discontinue a discipline
- Add or change a medication — including holding one
- Add a treatment, device, or piece of DME
- Change parameters for calling the physician
- Discharge before the certification period ends
You do not need an order to assess, to teach within the existing plan, or to report a finding.
New order versus clarification

The trap: writing a "clarification" that quietly adds something the physician never said. If the content of care changes, it is a new order, whatever it is called.
Taking a verbal order safely
- Get the right person. A verbal order comes from the physician or allowed practitioner — not the office staff relaying it. If the receptionist passes along a message, that is a message; it becomes an order when the practitioner agrees to it.
- Read it back. Every time. Drug name, dose, route, frequency, duration. Spell the drug if it sounds like anything else.
- Write it down while you are on the phone. Not afterwards.
- Record who, when, and what. Name of the person who gave it, date, time, and the exact wording.
- Enter it in the EMR the same day, and send it for signature.
- Tell whoever else is affected — the scheduler if frequency changed, the other disciplines, the aide if the aide plan changes.
Recording it in the chart
Whenever you receive a new order, write out all three:
- How you received it
- Who gave it to you
- What the order states
Example: received verbal orders from the office contact at Dr. Green's office, who stated to collect a BMP via venipuncture at the next SN visit on [date].
If there is no goal matching that order, write the order under the RN Case Manager's name and send it to them so the goals get updated.
If the patient hands you a signed copy from the doctor for labs or treatments, photograph it and file it under the RNCM's name so goals can be updated — adding the patient's name and date of birth if they are not already on it.
Writing an order that can be followed
An order has to be executable by someone who was not on your phone call.

Include: what, where, how, how often, for how long, and what would make you call.
Getting it signed
Orders need the physician's signature, and unsigned orders sitting in a folder are one of the most common survey findings there is. Track yours rather than assuming the office is chasing them.
When the physician will not order what the patient needs
It happens. What to do:
- Document what you recommended, when, and the response
- Escalate to your Field Supervisor — this is exactly what they are for
- Consider whether an MSW referral or a specialist is the better route
- Keep the patient safe within the orders you do have
Do not provide care outside the plan because it seems obviously right. If it is obviously right, the order will not be hard to get.
